Step 1

Initial assessment of your child's grade level or program applied for.

Step 2

Accomplish the online Application Form
Attach all SCANNED/CLEAR COPIES of the admission requirements

Junior Kinder and Senior Kinder Applicants

• Recent 2x2 colored picture with white background and name tag
• PSA Birth Certificate

Grade 1 to Grade 6 Applicants

• Recent 2x2 colored picture with white background and name tag
• PSA Birth Certificate
• Report Card with latest grades
• Certificate of Good Moral Character

Additional Requirements for Dual-Citizen Applicants

• Philippine Passport
• Foreign Passport
• Certificate of Recognition as Filipino Citizen
• Form 137 or School Permanent Record (if applicable)

Step 3

Pay the Application Fee of PHp200 and accomplish the Payment Confirmation Form.

Step 4

Oral Reading Assessment (Grade 3 to Grade 6 Applicants) and Issuance of Acceptance Letter

Grade 3 to Grade 6 applicants will be notified regarding the Oral Reading Assessment Schedule via email or messenger.

Applicants with complete requirements who paid the application fee and who have passed the Oral Reading Assessment (Grade 3 to Grade 6 only) shall receive the Acceptance Letter with a valid Student Number thru the email address provided by the parent/ guardian.

The UAGS Guidance and Admissions Office also accommodates Walk-in Applicants with Complete Admission Requirements every Monday, Wednesday, and Friday (8:00 am to 3:00
pm)
