APPLICATION INFORMATION

VENUE:

UAJHS Guidance Office
2nd Floor, Msgr. Ricardo Serrano Hall

TESTING DATES:

Every FRIDAY of November 2018 until March 2019
For April and May 2019, as scheduled upon admission depending on the available slots

ENROLLMENT DATES:

April 30, 2019 - Grade 7
May 2, 2019 - Grade 8
May 3, 2019 - Grade 9
May 4, 2019 - Grade 10

Admission Procedure

  1. The UAJHS Guidance and Admissions Office is a one (1) “STOP SHOP”. The applicant should secure an application form from the office located at the 2nd floor of the Msgr. Serrano Hall. The applicant is then given a slot for examination and pays the testing fee at the Treasury Service.
  2. The applicant must then take the entrance examination scheduled by the Guidance Counselor and must submit completely the following requirements:
    • Examination Permit
    • Two (2) Copies of Recent 2 x 2 ID Pictures (White Background with Name Tag)
    • Photocopy of latest Grade 6 Grades
    • Accomplished Application Form
    • Accomplished Recommendation Form
    • Photocopy and Original Copy of PSA Birth Certificate
    • Photocopy and Original Copy of Good Moral Certificate
    • Long Plastic Envelope
    • Official Receipt of Testing Fee (Php300.00)
  3. The whole morning will be allotted for testing of the scheduled applicant and in the afternoon, is the interview and discussion of the test results. The applicant will be interviewed by the members of the Admissions Committee. Then, the test results will be discussed to the applicant together with his/her parent/guardian. The applicant who passed the entrance test and interview will be given permit to enroll by the Guidance and Admissions Staff and can already pay the reservation fee of Php1, 000.00 to the Treasury Service to secure the applicant’s slot until the enrollment dates.

  8. Testing Fee (Php300.00)
